Access to Records & Consumer Rights

Clients may request copies of their health care records by submitting a written request to the practice at rebecca@innerspark-therapy.com. Requests are processed in accordance with Texas law.

 

Complaints about a licensee's ethics may be directed to the appropriate licensing authority:
