Patient Rights: Health Records & Complaints


In compliance with Texas Health and Safety Code Sec. 181.105, this page provides detailed instructions on how to request your health care records, contact our licensing authority, or file a consumer complaint.

1. How to Request Your Health Care Records
You have the right to access and copy your protected health information (PHI) maintained by this practice.

Step 1: Request the Authorization Form. Please email rebecca@innerspark-therapy.com.

Step 2: Complete the Authorization. Texas law requires written authorization. Ensure you specify which records you need and where they should be securely sent.

Step 3: Submit the Request. You can submit your completed form via the online Therapy Portal or via email.

Timeline & Fees: We process all records requests within 15 business days of receipt, as mandated by Texas law.

 

2. Licensing & Disciplinary Authority
As a Licensed Clinical Social Worker and Supervisor, my practice is regulated by the state of Texas. If you have questions regarding licensing standards or wish to contact the governing board, you may reach out to:

3. How to File a Consumer Privacy or Practice Complaint
If you believe your privacy rights have been violated under the Texas Medical Records Privacy Act, or if you have a complaint regarding professional services, you have the right to file an official report. Retaliation is strictly prohibited by law.

Option A: File a Complaint with TBHEC (Professional & Board Violations)
An individual who wishes to file a complaint against a behavioral health licensee may do so through TBHEC.

Option B: File a Privacy Complaint with the Texas Attorney General
The Texas Attorney General maintains a consumer information website to help patients report medical privacy and PHI violations.
